Doctors Referral Medicare Plans made easy

If you have had a chronic injury, you may be eligible for a Medicare Chronic Disease Management Plan.

Once you have a referral from your doctor, our physiotherapists will work with you and your doctor to create a plan for treatment and rehabilitation of your chronic injury.

What to do to get a medicare plan

If you have a chronic injury and your doctor thinks you are eligible, your doctor will refer you for a chronic disease management (CDM) plan. What to do next:

  • Get a CDM plan referral from your doctor.

  • Provide your doctor Game Changer Physiotherapy’s details.

  • Book an initial physiotherapy consultation online.

  • Bring your medicare card and referral letter to your initial appointment.

The majority of your appointment will be covered for up to 5 treatment sessions!
