If you have had a chronic injury, you may be eligible for a Medicare Chronic Disease Management Plan.
Once you have a referral from your doctor, our physiotherapists will work with you and your doctor to create a plan for treatment and rehabilitation of your chronic injury.
What to do to get a medicare plan
If you have a chronic injury and your doctor thinks you are eligible, your doctor will refer you for a chronic disease management (CDM) plan. What to do next:
Get a CDM plan referral from your doctor.
Provide your doctor Game Changer Physiotherapy’s details.
Book an initial physiotherapy consultation online.
Bring your medicare card and referral letter to your initial appointment.
The majority of your appointment will be covered for up to 5 treatment sessions!
